Shoulder MRI: Rotator Cuff, Labrum and What Else It Finds

For most rotator cuff questions, ultrasound is as accurate as MRI and a fraction of the price. Here is where each one actually wins.

A shoulder MRI shows the four rotator cuff tendons, the labrum, the biceps tendon, the subacromial bursa, the joint cartilage, the ligaments and the bone underneath. It is the most complete single look at the joint. For rotator cuff tears specifically, ultrasound is roughly as accurate and costs far less.

What a shoulder MRI shows

The rotator cuff is four tendons — supraspinatus, infraspinatus, subscapularis and teres minor — wrapping the top of the humerus. MRI shows each of them, and whether a tear is partial or full-thickness, how wide it is, and how far the torn edge has pulled back. It also shows the state of the muscle bellies behind them, including fatty change, which surgeons use to judge whether a repair is likely to hold.

The labrum, a rim of cartilage deepening the socket, is visible on MRI and essentially invisible on ultrasound. So are the joint surfaces themselves, the glenohumeral ligaments, and the bone marrow — which matters for fractures, bruising, avascular necrosis and bone lesions.

Around the joint, MRI reports on the subacromial-subdeltoid bursa, the acromioclavicular joint, the shape of the acromion, and the long head of the biceps tendon, including whether it is inflamed, torn or displaced from its groove.

Ultrasound versus MRI: the comparison worth making

This is where most people can save real money, so it is worth setting out properly.

A systematic review and meta-analysis by Farooqi and colleagues, published in the Orthopaedic Journal of Sports Medicine in 2021, compared ultrasound directly against MRI. For full-thickness tears, ultrasound had pooled sensitivity of 0.94 and specificity of 0.94, against MRI's 0.94 and 0.89. For partial-thickness tears, ultrasound scored 0.89 and 0.89, against MRI's 0.85 and 0.87. The authors concluded that "no significant difference was found for the sensitivity, specificity, or accuracy of US as compared with MRI", and pointed out that ultrasound is cheaper and quicker.

A more recent systematic review reached a similar place with an important caveat: ultrasound performs best on supraspinatus, the tendon most often torn, and less well on the other cuff tendons and on subtle partial tears, with results depending heavily on the operator's expertise and the equipment.

Ultrasound has one advantage MRI cannot match. It is dynamic — the sonographer can move your arm and watch the tendon glide, which is how impingement is assessed in real time.

Price makes the comparison sharper. In September 2026, a single-area ultrasound at UK providers ran from about £69 to £540 on published price lists, with several London specialists in the £175 to £295 range. The short version: if the question is "is the rotator cuff torn", a good musculoskeletal ultrasound often answers it. If the question involves the labrum, the joint surface, the bone, or planning an operation, MRI is the one that answers it.

What guidelines say about imaging shoulder pain at all

The Academy of Medical Royal Colleges' Evidence-Based Interventions guidance on scans for shoulder pain is clear that advanced imaging is not the starting point. For a first presentation in primary or intermediate care it states that "the first line of radiological investigation should be a plain x-ray", because X-ray picks up most routine shoulder problems including osteoarthritis, calcific deposits, cuff arthropathy and fractures. It goes on to say that "use of Ultrasound, MRI and CT scanning should be restricted to those secondary care services" — in other words, after a specialist is involved.

The Royal College of Radiologists takes the same approach across all imaging through its iRefer referral guidelines, which it has published since 1989 to help clinicians choose the right test for the question rather than the most detailed one available.

That does not stop you booking a scan privately. It does tell you what a well-run NHS pathway would have done first, and an X-ray at roughly £70 to £197 is a much cheaper way to answer some questions.

The honest part: torn cuffs are common and often silent

A mass-screening study of a whole village population, published in 2013, found full-thickness rotator cuff tears in 147 of 664 people — 22.1%. Of all the tears found, 65.3% were causing no symptoms at all.

Prevalence rose steeply with age: none in people in their twenties and thirties, 10.7% in the fifties, 15.2% in the sixties, 26.5% in the seventies and 36.6% in the eighties. In people over 60, roughly two-thirds of tears were asymptomatic.

The implication is the same as for knees and spines. If you are in your sixties and you scan your shoulder, a report describing a cuff tear is a fairly likely outcome — and it is not proof that the tear is what hurts. A clinician has to connect the image to your examination before it means anything.

When a shoulder MRI genuinely changes the plan

  • A shoulder specialist is considering cuff repair, and needs tear size, retraction and muscle quality.
  • Suspected labral injury — recurrent dislocation, a catching or dead-arm feeling in a thrower — which ultrasound cannot assess.
  • Pain and weakness persisting after a reasonable course of physiotherapy, with an unclear diagnosis.
  • Suspicion raised by a clinician of something other than tendon pathology: infection, inflammatory arthritis, a bone lesion.

If none of those apply, the honest answer is often that a physiotherapy course will do more for the shoulder than a picture of it.

Symptoms that need a doctor now

Some shoulder problems need seeing rather than scanning. A shoulder that is hot, swollen and painful with a fever. An arm you cannot lift at all after a fall, or an obviously deformed shoulder. New weakness or numbness spreading down the arm. Shoulder pain with chest tightness, breathlessness or sweating, which can come from the heart rather than the joint.
